The next step for the Weight Management Program is to complete an Intake Assessment so our Nutrition Team can review your medical history, goals and other information to make sure the program is suitable for you.
The Intake Assessment can be completed online and generally takes no more than 20 – 30 minutes to complete. Once you have completed your Intake Assessment for the Weight Management Program, our Nutrition Team will review your assessment and get back to you generally within two business days with your suitability for the program.
